At the Dahmen Family Foundation, we know how quickly life can change — and how much a little support can mean when it does. Founded in memory of Jolyn Dahmen, the foundation helps individuals and families facing illness and other hardships find their footing again.

If you’re going through something difficult, this short application lets us understand your situation and determine whether we may be able to help. Please answer as honestly and completely as you can. Every request is reviewed personally and with care.
